<p>The one-year-old girl died on December 1 in hospital, the health ministry said, adding that health officials discovered a strain of the (A)H1N1 virus resistant to Tamiflu in her body.</p>
<p>&#8220;It marked the first such fatality at home, but we need consultations with the (World Health Organisation) to see if it was the world&#8217;s first such case,&#8221; Chung Chan-Woo, an official at the ministry&#8217;s swine flu centre, told AFP.</p>
<p>The girl was hospitalised on November 14 for high fever and coughs but her condition continued to deteriorate despite double doses of Tamiflu, he said.</p>
<p>The girl was too young to be given another anti-viral drug, Relenza, which is approved for treatment only in children who are seven years and older, he said.</p>
<p>South Korea has reported two other cases of Tamiflu-resistant patients. Its death toll from swine flu has reached 118.</p>

<p>The Beijing Institute for Microbiology and Epidemiology has released sequences from a recent isolates collected in Beijing.  All eight were nasal swabs collected in November.  Although MP and NS sequences were deposited for all eight samples, these sequences match the pandemic H1N1 consensus and therefore were not informative.  However, NP sequences from five isolates had a number of polymorphic markers and all five sequences were virtually identical, These markers were found in other H1N1 sequences, but had a wide geographic distribution matching sequences from Texas (see list <a href="http://fluboard.rhizalabs.com/forum/viewtopic.php?f=5&amp;t=3814&amp;start=1">here</a>) , Spain (<a href="http://fluboard.rhizalabs.com/forum/viewtopic.php?f=5&amp;t=3814&amp;start=2">here</a>), and China (<a href="http://fluboard.rhizalabs.com/forum/viewtopic.php?f=5&amp;t=3814&amp;start=4">here</a>), as well as H5N1 (<a href="http://fluboard.rhizalabs.com/forum/viewtopic.php?f=5&amp;t=3814&amp;start=3">here) </a>or swine (<a href="http://fluboard.rhizalabs.com/forum/viewtopic.php?f=5&amp;t=3814&amp;start=5">here</a>) signaling extensive recombination.</p>
<p>Similar analysis for other genes showed the the Beijing sequences were closely related to each other, but the newly acquired polymorphisms traced back to H1N1 in a number of geographic locations.</p>
<p>However, most striking were the three HA sequences which had a clustering of polymorphisms flanking the HA cleavage site.  Several of these polymorphisms were non-synonymous and signaled rapid evolution of the HA gene.</p>
<p>Two of the non-synonymous polymorphisms (S131P and A337T) were in all three isolates (A/<a href="http://www.ncbi.nlm.nih.gov/nuccore/CY053338">Beijing/718</a>/2009. A/<a href="http://www.ncbi.nlm.nih.gov/nuccore/CY053348">Beijing/720</a>/2009, A/<a href="http://www.ncbi.nlm.nih.gov/nuccore/CY053353">Beijing/721</a>/2009).  Three were in 720 and 721 (T314P, L322M, S327F).  One was in 718 and 720 (S330Y).  Unique changes were in 718 (A319T), 720 (L5R), 721 (T320S, G342E, Y354F).</p>
<p>The large number of changes, and the combinations of shared polymorphisms, suggest significant recombination and rapid evolution, which may be heralding a new wave.</p>
<p>Full sequences on all eight isolates would be useful, as would more timely release of sequences from other countries.</p>

<p>More than 1,000 people showed up to the Round Lake Area Park District Sports Center to try for scarce H1N1 vaccine administered by the Lake County Health Department.</p>
<p>The H1N1 pandemic should have come with an announcement: this is a test of the public health emergency system.</p>
<p>If this had been a virulent virus, there&#8217;s a decent chance you might have been dead before you got the vaccine.</p>
<p>The H1N1 flu virus, also known as swine flu, turned out to be mild in most cases. But some experts say we dodged a bullet.</p>
<p>An estimated 50 million people got the flu &#8211; one out of six Americans. If the virus had been as deadly as first feared, the lack of vaccine would have created a national uproar.</p>
<p>The fact remains that an estimated 10,000 people in this country &#8211; including 79 in Illinois &#8211; died in part from flu complications because they did not get a vaccine.</p>
<p>And while health officials say supplies will increase rapidly in the next few weeks, it&#8217;s still hard to get.</p>

			<content:encoded><![CDATA[<div class='snap_preview'><br /><p>D225G and D225N In Utah Patient Raises Pandemic Concerns Recombinomics Commentary 23:55 December 6, 2009 The CDC has put HA sequences, A/Utah/42/2009, from a Utah patient (28F) on deposit at Genbank, which has D225G and D225N. Moreover, the sequence from the original sample gives mixed signals at tandem positions, revealing significant heterogeneity that the codon. A clone for the original sample has D225G. D225N and D225G have been identified in necropsy lung samples from Brazil (A Sao Paulo/53845/2009 and A/Sao Paulo/53838/2009 have D225N while A/Sao Paulo/53225/2009 and A/Sao Paulo/53206/2009 have D225G) raising concerns that these polymorphisms are associated with more severe disease.</p>
<p>The severity may be linked to viral load, because D225G and D225N were present as mixtures in early cases in the US in California and Texas. In fact the vaccine target, A/California/7/2009 was a mixture for D225G, as were several other isolates isolated at about the same time in Texas and California. Other isolates in the same areas, as well as New York, did not give mixed signals, but D225G was found in milder cases.</p>
